Medication Information

Does your child need medication during school hours?


New students will need to supply a copy of any health plans (Asthma, Diabetes, Allergy Action Plan or Anaphylaxis Plan) on the first day of school.

Other necessary paperwork can be downloaded from this website or collected from the Administration Office.

Please note - medications must be handed in by parents - students are unable to handle medications.

No longer need to take medication at school? 

Please advise the office via email asap so we can update our files and return unused medication to you for disposal.

Consent to Administer Medication

All students needing medication during school hours need to complete the "Consent to Administer Medication" form sign by the parent/caregiver.

This is an information document for doctors to compliment the Action Plan if required.

ALLERGY OR ANAPHYLAXIS​?​

If your student suffers from severe allergies or anaphylaxis, we require an updated (especially when moving from primary school to high school) Allergy or Anaphylaxis P​lan.

These are available from the below website and must be signed by a doctor.
