Your school vaccination provider, Logan City Council, is using a simple digital system for our School Immunisation Program. This online platform will streamline the process, making it quicker and easier for you to consent or decline participation for your child in the School Immunisation Program.
How it works:
• Use your phone, tablet or computer to follow the link to access the Vitavo parent portal: https://logan.book.vitavo.com.au/school/SpringwoodStateHighSchool-64MI7U
• Register to create a Vitavo profile for your child and provide consent for the vaccines being given for their year level. Your child will then be booked in to receive their vaccine/s at their scheduled school visit date.
• You can return to the portal at any time before the visit dates to confirm upcoming appointment dates or withdraw consent.
• If you do not want your child to be vaccinated, please select 'I do not consent' and prevent further follow-up from our team.
Electronic consent will be required for our upcoming visit on Friday 25 July 2025.

Important Notes:
• If you do not give consent for vaccinations at school, please still follow the link to submit the “I do not consent" electronically.
• If you only want to consent for one of the two vaccinations available, then please ensure you select the “I consent" option. This will then allow you to de-select the vaccine you do not wish to provide consent for.
• If your child is vaccinated at the school clinic on Friday 25 July 2025 you will be emailed the vaccination confirmation.
