Does your child need medication during school hours?
New students will need to supply a copy of any health plans (Asthma, Diabetes, Allergy Action Plan or Anaphylaxis Plan) on the first day of school.
Other necessary paperwork can be downloaded from this website or collected from the Administration Office.
Please note - medications must be handed in by parents - students are unable to handle medications.
New paperwork is required each year so even if you have already completed some, you will need to update it for 2025.
Consent to Administer Medication
All students needing medication during school hours need to complete the "Consent to Administer Medication" form sign by the parent/caregiver.
Request for Additional Medical Advice
This is an information document for doctors to compliment the Action Plan if required.
Record of Medication Administration (Routine Medication)
Record of Medication Administration (Insulin)
Record of Medication Administration ('As-needed' Medication)
The table below indicates which Authorisation form (Routine or as needed) you will require.
